On scaling, looks like madVR's GUI uses the "taps" descriptor for everything, regardless of correctness. If you're doing multi-channel LPCM over HDMI, you may be sending out more channels than your hardware can understand. If that doesn't fix it, hit up the mixing tab in LAV. Make sure your playback device in Windows' sound control panel has the right number of channels. Regarding weak dialog, it's likely something in the audio chain dropping channels rather than downmixing.

You may want to update your Intel/AMD chipset drivers as well. Make sure your motherboard firmware and sound card drivers are current, then go digging around the PCI (and USB if the sound card is USB-based) advanced power saving options if you still have problems. The audio breakup you're hearing on battery is likely a driver/firmware bug and/or heavy jitter on the bus while power saving, but that's just an educated guess.
